Divorce Documents Checklist

This is a list of the typical documents you need to file when starting your divorce case. Form numbers refer to forms that have been approved by the Florida Supreme Court, and they can be found for free at ​the Florida Court System's website.
  • Petition for Dissolution of Marriage (Form 12.901) 
  • Answer to Petition for Dissolution of Marriage (Form 12.903)
  • Vital Statistics Form
  • Notice of Related Cases (Form 12.900(h))
  • Affidavit of Military Service (If not covered in Petition) (Form 12.912)
  • Financial Affidavit (Form 12.902)
  • Certificate of Compliance with Mandatory Disclosure (Form 12.932)
  • Notice of Social Security Information (Form 12.902(j))
  • Notice of Confidential Information (to accompany Notice of  SSN)
  • Enlarged front and back copy of driver’s license or Affidavit of Corroborating Witness (Form 12.902(i)
  • If minor children:
    • If uncontested: Parenting Plan (Form 12.995),
    • UCCJEA (Form 12.902(d)),
    • Certificate of attending parenting class, and 
    • Child Support Guidelines Worksheet (Form 12.902(e))
  • Designation of Address (Form 12.915)
  • If uncontested: Marital Settlement Agreement (Form 12.902(f))
  • Final Judgment (3 copies) (Form 12.990)
  • Special Interrogatories (if uncontested, and only in some Florida courts)
