What Documents Must You Provide Under The Mandatory Disclosure Rule?
Florida Rule of Family Court Procedure 12.285 governs the Mandatory Disclosure requirements.
Rule 12.285 requires both spouses to file Financial Affidavits.
In addition to exchanging Financial Affidavits, Rule 12.285 requires the exchange of these additional documents:

In addition to exchanging Financial Affidavits, Rule 12.285 requires the exchange of these additional documents:
- Personal Federal and State Income Tax Returns for the preceding 3 years (including all attachments)
- Gift Tax Returns and Intangible Personal Property Tax Returns for the preceding 3 years
- IRS Forms W-2, 1099, and/or K-1 for the past 2 years, if your income tax returns have not yet been prepared or filed for the previous 2 years
- Pay Stubs and similar evidence of Earned Income over the past 6 months
- A statement identifying the amount and source of all income received from any source during the previous 6 months
- Loan Applications prepared or used for any purpose within the past 24 months
- Deeds for any real estate in which you presently own or have owned an interest within the past three (3) years
- All promissory notes in which you presently own or owned an interest within the last 24 months
- All present leases in which you own an interest, in which you received any payments within the last three (3) years, or in which you are the person leasing the property
- Checking account statements for the last twelve (12) months
- Savings accounts, money market, certificates of deposit, etc. statements for the last twelve (12) months
- Brokerage Account statements for the last twelve (12) months
- Retirement Plan Statements for the last twelve (12) months
- Statements for any virtual currency transactions for the last twelve (12) months
- The declarations page, statements for the last twelve (12) months, and Certificate for any life insurance policies insuring your life or the life of your spouse
- A copy of health and dental insurance cards covering you, your spouse and/or your dependent child(ren)
- Corporate, partnership, and trust tax returns for the last 3 tax years, in which you have an ownership or interest
- All credit card statements (and statements for other loans) over the past 24 months
- All promissory notes on which you presently owe or owed within the past 24 months
- Any Premarital and Marital Agreements Between you and your spouse
- All affidavits and declarations of non-paternity or judgments of disestablishment of paternity for any minor or dependent children born or conceived during the marriage
- All documents supporting the producing party's claim that an asset or liability is nonmarital, for enhancement or appreciation of nonmarital property, or for an unequal distribution of marital property. The documents produced must be for the time period from the date of acquisition of the asset or debt to the date of production or from the date of the marriage, if based on premarital acquisition.
- Court Orders directing you to pay or receive Child Support and/or Alimony
- Any additional documents you intend to use at trial or hearing to support how you are requesting the Judge to divide your marital property, especially if you are requesting an unequal division of property, or if you are claiming property is non-marital
